• New Customer Application  All applications require completion of the first two pages. If applying for credit terms, fill out all four pages.
• IRS Form W-9
• Texas Comptroller Form 01-339 if you choose to make non-taxable purchases, or to claim an exemption from payment of sales and use taxes
